I use wax paper, a long knife, and have had a ton of practice. I think the frosting recipe you use plays a big part in how smooth you can get a cake. I use a basic white icing and the old Wilton buttercream recipe. With both you don't have to frost the cake perfectly smooth because the wax paper will get rid of a ton of imperfections.
